Dr. Avesh Thaker

Avesh Thaker, D.O., is a board-certified internal medicine physician and hospitalist with broad experience in inpatient medicine, quality improvement, clinical documentation, and hospital operations. Since 2022, he has served as an Internal Medicine Hospitalist at OSF Little Company of Mary Hospital, providing daytime and swing-shift coverage, utilizing Epic EMR, and conducting weekly pre-operative evaluations for surgical clearance. He also developed standardized documentation templates to improve accuracy, efficiency, and compliant billing practices.
Dr. Thaker has held leadership roles in quality and patient safety initiatives. As a member of the hospital’s Quality and Safety Committee since 2024, he has monitored CLABSI, CAUTI, and C. difficile rates, collaborated with Infection Control teams, and helped implement policies to reduce preventable complications. Since 2025, he has also served on the Hospital Readmission Reduction Program, focusing on CHF and COPD patients. Through bedside medication delivery, expedited Heart Failure Clinic follow-up, smoking cessation counseling, inhaler education, and coordinated pulmonary care, he helped reduce readmission rates from 20% to 10%.
In addition to his primary role, Dr. Thaker has worked as a daytime moonlighting hospitalist at St. Mary of Nazareth Hospital since 2025, where he teaches and mentors internal medicine and family medicine residents through bedside instruction, lectures, and clinical supervision. He emphasizes the direct connection between documentation quality, patient care, and outcomes, and advocates that effective documentation should be accurate, concise, and clinically meaningful rather than excessive. His teaching reinforces CMS-compliant documentation standards and the importance of strong documentation practices as a core competency in residency training and clinical medicine.
Dr. Thaker completed his Internal Medicine Residency at Franciscan Health St. James Hospital from 2019 to 2022. He earned his Doctor of Osteopathic Medicine degree from Midwestern University Chicago College of Osteopathic Medicine and graduated magna cum laude with honors in Chemistry and Mathematics from Loyola University Chicago.
